Dr. Charles Bowkley III M.D. is an Interventional Radiologist who specializes in the diagnosis and treatment of varicose veins. A consultation with our physician and a simple ultrasound can diagnose venous insufficiency.
A little background on how the veins work, well how they’re supposed to work. In normal veins, valves open and close to get blood back to your heart. Venous insufficiency, the cause of varicose veins, happens when the valves inside the veins fail and let blood flow in the wrong direction.
There are several factors that increase your chances of developing varicose veins:
Those who have had a deep vein thrombosis (DVT) are also at risk for a similar problem known as deep venous reflux.
If left untreated, varicose veins can lead to skin ulcerations. These are wounds in the area of the calf and ankle that do not heal without treating the underlying vein. Often, these wounds can become infected, warranting immediate intervention.
The ultrasound test takes about an hour. The technologist will squeeze your legs while documenting the direction of blood flow in the veins. Once we have the results of the ultrasound you and your physician will work together to choose the best plan of care.
Diagnosis of venous insufficiency and its treatment are considered medical conditions, and they are covered by most insurance companies. Our billing office takes care of the pre-certification process for you.
Our success rate is nearly flawless. With proper post procedure care, recurrence rates in the treated veins are almost zero.

At Wyoming Vein Specialists, your consultation and diagnostic imaging are catered to your specific presentation. This process starts with one of our registered diagnostic sonographers performing an ultrasound study assessing your deep venous system for any abnormalities. The next portion of the examination requires that are standing upright during which we will gently squeeze your legs in order to assess the function of the valves in your veins. Sonographic venous mapping is a painless procedure and usually takes anywhere from one to one and a half hours. The examination may take longer depending on the complexity of your vascular system. Once the mapping is complete, our board-certified interventional radiologist will review your images with you and determine what treatment is most appropriate. You will leave your appointment with your questions answered and a plan of care in place.
Wyoming Vein Specialists will submit all the necessary documentation to your insurance company to obtain approval for the procedures needed. The majority of insurance companies, including Medicare and Medicaid, cover services related to superficial venous disease. Authorization can take up to 7-10 days. Once your insurance has approved your plan of care, our team will work with you to determine a time and date that you can receive treatment that works best for your schedule.

Sclerotherapy is the treatment of choice for small varicose veins and spider veins. There are many different types of sclerosant medications. These medications are used to irritate the inside of your veins so that when we place you in special fitted compression hose, the vein walls will adhere to one another and blood will stop flowing in the wrong direction. We begin by assessing the vein (or veins) to be treated with ultrasound. Once the appropriate vein is identified, we clean the treatment region with alcohol. A small sterile needle is used to inject the medication into the appropriate vein/veins. This results in the irritation of the vein wall. Compression hose are applied immediately following the injection in order to promote closure of the injected vein/veins. In order to achieve the best results, sclerotherapy may require more than one treatment session. Following the injection(s), you will remain in compression hose for a 14-day period. A follow up appointment will be scheduled between 4 and 6 weeks after your treatment.

Endovenous laser ablation is a minimally invasive procedure that uses heat to treat varicose veins. One of our board-certified interventional radiologists performs this procedure, and in our hands is 99.9% safe and effective. The procedure is performed using sterile technique. Betadine or chlorhexidine are used to cleanse the skin. The vein we intend to treat is then identified using ultrasound. Once this is confirmed, the venous access point is numbed using lidocaine (yes…. pinch, burn, ouch – but this only lasts for 30-60 seconds), a small nick is made in the numb area of skin, and a catheter is placed into the vein. Movement of the catheter inside the vein is typically not felt by the patient. The laser fiber that distributes heat to treat the vein is advanced to the safe and appropriate location. Once the laser is in place, our board-certified interventional radiologist will use more lidocaine to numb around the vein so you remain pain free throughout the procedure (Most patients say that this part feels “weird” or like “pressure” along the length of the vein.). Next, the laser fiber is turned on and heat is distributed along the vein, causing the walls of your vein to become irritated so that when we place you in special fitted compression hose, the vein walls will adhere to one another. This will prevent blood from flowing in the wrong direction, which is the cause of the symptoms you are having! A small bandage is applied over the catheter insertion site and you are typically placed in compression hose for a 14-day period. We will call you the following day to see how you are doing and an office visit will be scheduled between 4 and 6 weeks after your treatment.

Venaseal is a type of pharmaceutical grade glue, called Cyanoacrylate that can be used to treat the cause of varicose veins. This procedure is performed by one of our board-certified interventional radiologists. The procedure is performed using sterile technique. Betadine or chlorhexidine are used to cleanse the skin. The vein we intend to treat is then identified using ultrasound. Once this is confirmed, the venous access point is numbed using lidocaine (yes…. pinch, burn, ouch – but this only lasts for 30-60 seconds), a small nick is made in the numb area of skin, and a catheter is placed into the vein. Movement of the catheter inside the vein is typically not felt by the patient. The catheter tip, where the glue is administered, is advanced to the safe and appropriate location. Catheter placement is confirmed with ultrasound. Gentle pressure is applied to the vein where the glue is administered. As the catheter is slowly removed from the body a small amount of glue is deposited along the abnormal vein. Closure of the vein is typically immediate and confirmed with ultrasound. Closure of this vein is what will prevent blood from flowing in the wrong direction – the cause of your symptoms! A small bandage is applied over the insertion site. Once the vein is treated the limb is cleaned and a small bandage is placed over the insertion site. No compression hose are necessary following this procedure. We will call you the following day and we will schedule an office visit between 4 and 6 weeks after treatment.

IMPORTANT: Patients with allergies to adhesives, tape, or glue cannot safely receive Cyanoacrylate and therefore are not candidates for Venaseal treatment.

A clinical and ultrasound follow up to your treatment is absolutely necessary and your appointment is usually scheduled between 4-6 weeks after your procedure. We will use ultrasound to assess the efficacy of your treatment. This is performed by our registered diagnostic sonographers who have undergone in-depth training in vascular imaging. Our sonographers will ask you a few questions to determine and help refine their evaluation based on your progress since your procedure. The sonographer performing your exam will first take a look at your deep venous system. Next, they will evaluate the closure of the treated vein. Images are taken throughout this process. Our board-certified radiologist will then review these images and discuss the findings with you.
